Fallout 4 is finally happening according to the NDA(Non-Disclosure Agreement) takedown notice by Mirada Studios (The studio marketing studio behind the upcoming Fallout 4 trailer).

Destructoid reports that Mirada Studios, as cited a breach of a non-disclosure agreement and demanded the site remove its coverage of the artist’s profile. Destructoid says it never signed an NDA about this stuff, but since an NDA apparently exists, that must mean the trailer does as-well.

This could just be speculation. However I personally think that we’ll be seeing some Fallout 4 at Bethesda’s first ever E3 showcase this year.
